Our Blood Test Clinics are now appointment ONLY. Please contact the surgery for more details or to make an appointment. Or patients can still attend the phlebotomy clinic at Croydon University Hospital which operates a walk in clinic 7am  - 4.45pm Monday - Friday and with late opening till 6.45pm on Tuesday and Thursday.

Flu Clinics - Our flu clinics are now available. The children's nasal flu vaccine for 2 and 3 year olds is now in stock. Please make an appointment in one of our nurse flu clinics. 

 

Pre-Registration with the practice

If you wish to pre-register click on the link below to open the form. When you have completed all of the details, click on the "Send" button to mail your form to us. When you visit the surgery for the first time you will be asked to sign the form to confirm that the details are correct.

Please be aware that completion of the online pre-registration form does not mean that you will be automatically registered with the practice. To complete the registration process you will need to provide 2 forms of identification one of which MUST confirm your address is within our catchment area. For example:

  • Passport or National ID card
  • Photobearing driving licence
  • bank/credit card
  • NHS number card
  • Recent bank statement
  • Recent utility bill
  • Tenancy agreement or mortgage statement
  • council tax bill
  • Recent correspondence from a government body

If you are unable to produce any of these documents then you may only be registered temporarily for emergency i.e. life threatening, or immediately necessary treatment.


Pre-registration Form

When you register you will also be asked to fill out a medical questionnaire. This is because it can take a considerable time for us to receive your medical records. There is an online version of this file too, which you may fill out and send to us. When you come to the surgery you will be asked to sign this form to confirm that the details are correct.

Note that by sending the form you will be transmitting information about your self across the Internet and although every effort is made to keep this information secure, no guarantee can be offered in this respect.

 


Alternatively you may print off a registration form, fill it out and bring it in with you on your first visit to the practice.
